This allows your practitioner to understand your health concerns and recommend the most appropriate treatment plan for your needs.

If you have already completed your initial consultation and are aware of your practitioner's recommendations, you may submit your details here and mention that you are booking a follow-up appointment. Dr. Sonia will then contact you to arrange the next steps.
